If the vet cant figure out why, maybe you should consider switching vets. As a vet tech, I know skin diseases are a very common problem with animals. It could be caused from food or perhaps something in the environment.
What kind of food does your dog eat? There is a hypoallergenic diet your vet should know about that is fed for 3-4 weeks. If symptoms improve during this time, it could diagnose a food allergy.
It could also be an inhalant allergy. It's diagnosed in the vet clinic by ID testing or serum tests.
There are different antibiotics, antifungals, & steroids available as well as shampoos that can treat skin problems. Your vet should be able to steer you in the right direction.

has your vet not suggested any possible causes at all? if not I agree you need a new vet.
altho most vets don't want to admit it, probably at least 75% of skin problems, ear infections, hair loss & other conditions are food based allergies.
vets get no training in school on nutrition so they push what they get best kick backs on. the brands most of them sell are garbage.
before you spend a fortune on tests - what exactly are you feeding? what are you using for treats? is your dog on a flea preventative? have you checked closely for fleas & flea dirt
